8e2b13386b ENH: enable 'status=done' when finishing an externalCoupled FO 
- This provides a mechanism for the external code to detect when
  OpenFOAM is done.

- Adjust tutorial to use the mechanism. Also test in parallel.

8d5800c5f8 ENH: externalCoupled: changed to non-OutputFilter functionObject. 
- moved control to functionObject (from bc)
- this allows multi-region support
- see heatTransfer/chtMultiRegionFoam/externalCoupledMultiRegionHeater tut
- generalisation of streamed reading/writing of specialised bcs
