Mark Olesen b2bf82360c COMP: cleanup of surfaceCoarsen/bunnylod
- remove MSWindows components (code and makefiles)

- use cxx, hxx extensions to reduce possible confusion with other
  "list" and "vector" classes

- use C++ versions for C headers
